Upcoming Presentations
HCPF is introducing an updated primary care alternative payment model structure for all Primary Care Medical Providers (PCMPs). This model integrates design elements that previously operated as separate programs managed by HCPF and the Regional Accountability Entities (RAEs). The updated, blended model simplifies the program while incorporating pediatric considerations. The result is a singular, comprehensive Primary Care Payment Structure that aims to elevate the quality of primary care services provided to Health First Colorado members.
HCPF is inviting PCMPs for a series of virtual education sessions on the new Primary Care Payment Structure. These sessions will include program updates to value based care programs for PCMPs and ACC Phase III quality programs. Materials from an introductory session held November 2024 are available under the “Past Presentations and Materials” drop-down below.
Pre-registration is required - you may register for one or multiple sessions at a time through Zoom:
- Session 1: Medical Home Payment: Thursday, March 27, from 12:00 to 1:30 p.m.
- Session 2: Quality Payments: Wednesday, April 9, from 12:00 to 1:30 p.m.
- Session 3: Primary Care Services Payments: Wednesday, April 23, from 12:00 to 1:30 p.m.
- Session 4: Shared Savings – Wednesday, May 7, from 12:00 to 1:30 p.m.
Please note topics may shift as needed. If planned discussion topics change, updated information will be sent via email. If you are unable to attend, a recording of the sessions will be available on this webpage.
