Now that we’ve moved past the initial transition period to Phase III of the Accountable Care Collaborative (ACC), we have discontinued the form to submit critical issues from this change. If you need assistance, we recommend the following steps: 

  • Providers: For questions or concerns about contracting, payment, attribution or member care coordination and support, contact your Regional Accountable Entity (RAE). If you have already reached out to your RAE and have further questions or require additional support, please see the Provider Help webpage.
  • Members: If you work with members, please share the following resources:
    • For general questions about Health First Colorado (Colorado’s Medicaid program), contact the Health First Colorado Member Contact Center at 1-800-221-3943 (State Relay 711).
    • To change your primary care provider or opt out of a managed care plan, contact Health First Colorado Enrollment at 1-888-367-6557 (State Relay 711).
    • For help finding a provider or coordinating care, contact your Health First Colorado regional organization. You can view your regional organization in PEAK or the Health First Colorado mobile app.

About ACC Phase III

The Department of Health Care Policy and Financing (HCPF) administers Health First Colorado (Colorado’s Medicaid program), Child Health Plan Plus (CHP+) and other health care programs for Coloradans who qualify. Created in 2011, the ACC is the primary delivery system for Health First Colorado. Regional Accountable Entities (RAEs) are responsible for promoting member health and well-being by administering the capitated behavioral health benefit, establishing and supporting networks of providers and coordinating medical and community-based services for members in their region. Two physical health managed care capitation plans, referred to as Managed Care Organizations (MCOs), deliver physical health care in certain areas of the state. New contracts with RAEs and MCOs, ACC Phase III, launched July 1, 2025.

ACC Phase III has been designed to achieve the following goals:   

  • Improve quality care for members
  • Close health disparities and promote health equity for members
  • Improve care access for members
  • Improve the member and provider service experience
  • Manage costs to protect member coverage, benefits, and provider reimbursements

Meet the ACC Phase III RAEs and MCOs

In September 2024, HCPF announced the awards for the four RAE contracts. HCPF is pleased to announce that, beginning July 1, 2025, the following entities have been awarded contracts. For information about the current RAEs, please visit the ACC Phase II webpage.

ACC Phase III Stakeholder Engagement

We are grateful to everyone who has provided feedback through the ACC Phase III Stakeholder Engagement process. A full archive of stakeholder engagement activities and associated materials are available on the ACC Phase III Stakeholder Engagement webpage.

From October 2022 through March 2024, there were 135 stakeholder presentations, 13 of which were members-only, with 5,732 attendees (this includes participants who attended multiple presentations). HCPF also hosted 10 surveys with 489 total responses, 55 from members and 208 from providers. Over this period, HCPF had, on average, 3,113 subscribers to the monthly ACC newsletter with a 37% open rate.
