Accountable Care Collaborative Phase II

The Department of Health Care Policy & Financing (HCPF) is committed to creating a high-performing, cost-effective Medicaid system that delivers quality services and improves the health of Coloradans. Created in 2011, the Accountable Care Collaborative (ACC) is the primary delivery system for Health First Colorado. Regional Accountable Entities (RAEs) are responsible for promoting member health and well-being by administering the capitated behavioral health benefit, establishing and supporting networks of providers and coordinating medical and community-based services for members in their region. 

As of July 1, 2025, a new region map and new contracts with the RAEs, referred to as ACC Phase III, have launched. See the ACC Phase III webpage for current program details. This webpage contains program details for ACC Phase II, the period from July 1, 2018, to June 30, 2025.

HCPF has published and will regularly update information on the Provider and Stakeholder Resource Center. This resource center includes fact sheets, FAQs and other resources for providers and stakeholders.

Member Messaging Resource Center

HCPF has developed and will regularly update the Member Messaging Resource Center. This resource center includes messaging for Health First Colorado members that can be used by our partners including sample letters, talking points, frequently asked questions, and more‚ all developed with our Health First Colorado members in mind. We ask our partners to use these resources when communicating with Health First Colorado members about the ACC.
