Home Delivered Meals are available to eligible waiver members who have been discharged from the hospital, are transitioning into the community, or are experiencing a qualifying change in life circumstances.
The Home Delivered Meals (HDM) benefit is available on the following Home and Community-Based Services (HCBS) Waivers:
- Brain Injury Waiver (BI)
- Community Mental Health Supports Waiver (CMHS)
- Complementary and Integrative Health Waiver (CIH)
- Developmental Disabilities Waiver (DD)
- Elderly, Blind and Disabled Waiver (EBD)
- Supported Living Services Waiver (SLS)
Who is Eligible?
Qualifying members are:
- Returning home from a hospital following a 24-hour stay, or
- Transitioning from an institutional setting (like a Nursing Facility) to a community setting, and enrolling on an adult waiver, or
- Experiencing a qualifying change in life circumstance.
- Examples include but are not limited to:
- Loss of member’s primary caregiver
- Member has recently aged out of the Medicaid programs for children
- Member is moving out of parent’s home to live independently in their own apartment

Benefit Information
Q: How many meals may I receive?
- A: You can opt to receive up to 2 meals per day.
Q: How long may I receive meals?
- A: Members discharged from a hospital may receive meals for up to 30 days. Members transitioning into the community or experiencing a qualifying change in life circumstances may receive meals for up to 365 days.

Q: May I choose who provides the meals?
- A: There are several enrolled home-delivered meal providers to choose from. Contact your case manager to get an updated list of providers.
