Find Your Provider Type

Enrollment requirements will vary based on the provider type. Please review the provider types available using the buttons below. Some provider types will need to submit an application for both their service location and their individual enrollment.

If enrolling more than one GROUP OR FACILITY location or Provider Type, a separate application for each is required. An individual enrolling via SSN is restricted to one enrollment.

If waiver services are provided, select the Home and Community-Based Services (HCBS) Providers button for the list of services covered. HCBS is provider type 36.

Terms and Definitions

RISK LEVEL

Each provider type has been designated limited, moderate, or high risk.

  • Limited = meets Federal/State requirements, license/certification verifications, database checks, etc.
  • Moderate = limited requirements plus a site visit at the service location address.
  • High = moderate requirements and fingerprint criminal background checks.

FEE REQ'D?

The provider type may be required to pay an application fee as required by Federal regulation.

NPI REQ'D?

The provider type may be required to have a National Provider Identifier (NPI).

MEDICARE REQ'D?

The provider type may be required to enroll as a Medicare provider before enrolling as a Health First Colorado provider.

OOS ALLOWED?

(Out-of-state (OOS) enrollment allowed?) Some provider types allow for out-of-state service locations to enroll.

BT ALLOWED?

(Border town (BT) enrollment allowed?) the provider type allows enrollment when the service location is in an approved border town. Please see Appendix F for a list of approved border towns.
