Transition Process

In-Reach Counseling

In-Reach Counseling is a service designed to engage and provide members living in a nursing facility with information regarding housing, transition, and other community-based services available to support them in living in the community and answer any questions.  In-Reach Counseling provides the referral process to transition coordination and other services while supporting members with making an educated choice about their options for where they live. 

In-Reach Counseling comes in two forms, Group and Individual. Group In-Reach is conducted in a group setting while Individual In-Reach is conducted one-on-one with a member and their chosen supports, if needed.

Group In-Reach Counseling provides a referral process to either Options Counseling or Transition Coordination Services while supporting members with making an educated choice about their options for where they live.

Options Counseling

If you need or want more information, a meeting for Options Counseling is scheduled.

Options Counseling is a person-centered approach to help you gain an understanding of the benefits and limitations of long-term services and support options.  An Options Counselor will educate you on these services through a personal interview, help you identify your needs, provide benefit counseling on both private and public programs and services, and assist you in evaluating the benefits and disadvantages of each.   

An Options Counselor can assist you in applying for benefits and make a referral to a Transition Coordination Agency.

Transition Coordination

If you are ready to move forward with transitioning, a referral to a Transition Coordination Agency (TCA) is made.

Transition Coordination includes meeting with a Transition Coordinator who will research necessary supports and services to see what is available in your community, create a plan for transition and moving day, and provide ongoing support after the transition.

You and your transition coordination team will discuss how you will be best supported for transition into living in the community, this could include housing.

Facility-Assisted Transition

If you are interested in transitioning from a nursing facility you are not required to use any of the above services. 

You can speak with the Social Service Director or Administrator at your nursing facility and they can assist you with a transition into the community without additional support from Transition Coordination.
