Prenatal Plus Providers

The Prenatal Plus (PN+) program is an enhanced program for pregnant Health First Colorado (Colorado’s Medicaid program) members, who are at risk for negative maternal and infant health outcomes due to lifestyle, behavioral, and socio-cultural risk factors. 

Prenatal Plus provides a multidisciplinary team-based approach that provides care coordination, nutrition services, and mental health counseling in addition to routine prenatal medical care during the perinatal period. 

The Department of Health Care Policy & Financing (HCPF) encourages providers to refer high-risk, pregnant Health First Colorado members to a local Prenatal Plus provider or to start their own Prenatal Plus program. 

How to Apply to become a PN+ Provider

Once your site has identified your care team members,  you may fill out a PN+ application. The program provides eligible members with:   

  • A Colorado Medicaid-enrolled physician, nurse practitioner, certified nurse-midwife, or physician’s assistant who is the rendering provider that delegates the provision of Prenatal Plus Program services to the multi-disciplinary team
  • A registered dietician who can provide pregnant members with nutrition education and Supplemental Nutrition Program for Women, Infants, and Children (WIC) support.
  • A mental health professional who can address their psychosocial needs and increase resiliency.
  • A care coordinator to provide members with support throughout the pregnancy and advocate for services based on their individual needs.
