Nursing Facility Transition and Diversion Stakeholder Engagement

There are a variety of way you can be involved as we make policy changes, procedure updates, and rule revisions.  The engagement of members, families and stakeholders throughout these changes is a critical part of this process.

Nursing Facility Open Office Hours

These monthly meetings provide information to nursing facility social workers to help them navigate the community transition process and provide information about available programs, services, and statewide resources.

 

Rule Review Meetings

Rapid Reintegration Rule Revision Workgroup 

This workgroup invites case managers, transition coordinators, and other interested stakeholders to collaborate and advise the Department of Health Care Policy and Financing (HCPF) as it creates regulations for Rapid Reintegration. 

Rapid Reintegration is the person-centered process that occurs when an individual who is seeking admission to a nursing facility, is interested in, or does not oppose, living in the community. This rule revision is necessary to ensure compliance with the Department of Justice agreement while providing individuals with consistent information about services and supports in a timely manner.

HCPF invites you to attend one of the following opportunities to discuss Rapid Reintegration and rule language, outline the process, and provide basic information about what stakeholders can expect moving forward.  The same information will be discussed at both meetings, therefore, you only need to attend once.
