Continuous Improvement Team

The Continuous Improvement Team has been designed by Health Care Policy & Financing (HCPF) to support all Eligibility Sites (Counties, MA sites, and Eligibility Assistance Partners) in the State of Colorado in the area of Continuous Improvement (CI). The Continuous Improvement Team is part of the Performance Improvement Team (PIT) which belongs to the Operations section within the Eligibility Division.

The Continuous Improvement Team developed the “Performance Coaching Workbook” to support Colorado Eligibility Sites in performance coaching & performance improvement.

The goal of the workbook is to support Eligibility Sites in understanding how to improve performance by coaching to key performance indicators (KPI’s) such as timeliness and quality assurance.
